Community Service Form

All CHS students are required to complete 20 hours of community service as stated in Board Policy 5122 as a graduation requirement.  Community service forms can be printed from this website (link below). When hours are completed, please scan and email the form to Ms. Sharma at vineeta.sharma@carlsbadusd.net. Be sure to scan a business card, or a letter from the service organization on their letterhead.  Your hours will then be recorded. Please keep this form for your records.

Community service can be completed by working without pay for a non-profit organization that benefits your fellow man, the environment, or the animal kingdom.  If you are paid for a service, it is not considered community service.  Fundraising for a team is also not considered community service.
